I have had several patients ask me to discontinue their nitrates (including ISDN) so that they could get sildenafil. I work in the VA system where there is scrutiny of all rx'd meds; you can't prescribe sildenafil to someone on nitrates. Ha ha ha. Many on sildenafil are so focused on getting an erection and would not care if you told them that it might cause blindness. I have had several patients ask me to discontinue their nitrates (including ISDN) so that they could get sildenafil.
